We are so excited to help you on your journey to providing a CHAMPS Camp in your area! CHAMPS Camp is a special program developed by a team committed to improving the lives of children with CHD and their families.
CHAMPS Camp has truly changed the landscape for children with heart conditions and their families in Saskatchewan, and we hope it can have the same impact in your community!
STEP 1
To begin, please fill out the "New Site Interest Form" below. This will help our team assess your resources and needs.
STEP 2
The Saskatchewan Camp Coordinator will contact you to set up an initial meeting after submitting the form. At this meeting we will start to lay the foundation for your program!
Note: filling out the form and participating in the initial meeting does not formally commit you to starting a CHAMPS program.
STEP 3
Ongoing meetings, discussions and planning to help you develop the optimal program for your region!

Start your pilot program!
